Registration Document for Biohazards (New Protocols)
Please complete this form prior to acquiring and handling biohazardous materials including, but not limited to,
- human and non-human primate derived materials such as blood and blood products, tissues, body fluids, primary or established cells lines (including those obtained from commercial sources),
- bloodborne pathogens,
- animal and plant pathogens,
- toxins of biological origin,
- infectious microorganisms,
- recombinant or synthetic nucleic acid molecules, and
- select agents.
Please complete this form prior to implementing changes to your current study including, but not limited to,
- personnel changes,
- addition of new biohazardous materials,
- addition of living organisms,
- addition or changes to the technology or methods used such aerosol-generating procedures, culturing microorganisms, growing microorganisms, and/or
- any other changes that may increase the risk of exposure to project personnel or cannot be performed within containment (i.e. biological safety cabinet).

Please complete this form prior to transferring biological materials between 51³Ô¹ÏÍø laboratories. The IBC will review and ensure appropriate safeguards exist in the recipient's lab and a current Registration Document for Biohazards is on file prior to approving the transfer.
